INDOOR VISITATION INFORMATION:

Updated October 16, 2020: All outdoor and indoor visits are suspended until October 29, 2020 following a positive Carepartner test result. Please feel free to reach out to your teams to reschedule your visit, however please be aware that should we receive another positive test result visits will once again be canceled. Thank you for your understanding!

As you are aware, we have received new state orders regarding visitations within the facility. The details below will outline the guidelines set forth by this new order:​

  • Facilitating visitations via phone or electronic communication, such as Skype.

  • Allow in-person visitation ONLY for the following circumstances:

    • The visitation supports the need for visitor assistance with activities of daily living (ADL’s) for Residents with such impairments as hearing, vision, and speech. 

    • A change in Resident’s condition that could be improved with the support of the visitor (ex: feeding to encourage adequate nutritional intake).

    • Residents who are receiving hospice services.

    • Residents receiving end of life care who are not enrolled in hospice services as determined by the Interdisciplinary Team (IDT).

    • A Resident whom a treating medical professional has deemed their well-being is at risk and a visitor could be a potentially effective intervention as determined by the Interdisciplinary Team (IDT).

    • A Resident who is experiencing a significant change in condition (decline) from baseline as determined by the Interdisciplinary Team (IDT).

​Visitations will be permitted with the following rules set in place:

  • Visits will take place by appointment only and will be limited to the number of visitations happening at one time. 

  • Visits will have a 20 minute time limit to follow and will require arrival and departure times to be logged.

  • Visitors will be limited to no more than two at a time.

  • All visitors will be screened upon arrival which will include the following:

  • Temperature and answering a series of questions regarding symptoms and possible exposure.

  • Personal protective equipment (PPE) MUST be worn throughout the visit which could include all of the following: Gloves, Gown, Mask, and Face Shield.

  • Social distancing of all visitors will be required when interacting with staff and Residents unless providing direct ADL assistance. This means remaining 6 feet from one another and no physical contact. 

  • All visitations will be supervised by a staff member trained in infection control procedures.

  • Visitors will not be allowed to share food or drink with Resident. 

  • Visitors should not bring items in for the Resident on the day of their visit. 

  • Visitors should only bring essential items into the facility. Items such as car keys and cell phone all other items should be left locked in their vehicle. 

  • Visitors should use the restroom prior to coming as the restroom will not be available during their visit. 

  • Pets are not allowed to visit.

Visitations will NOT be permitted under the following circumstances:

  • The visitor is under the age of 18. 

  • The Resident is in isolation or under observation for symptoms of COVID-19

  • The visitor has symptoms consistent with COVID-19. If this is the case and the visitor has visited the facility within 14 days prior to symptom onset, the visitor will notify the facility promptly.

  • The visitor is unable to cooperate or tolerate wearing the required PPE. This means all visitors must be able to medically tolerate wearing a surgical mask over their mouth and nose for at least 20 minutes. If they cannot medically tolerate it, in-person visits are not an option. 

  • The visitor does not comply with the guidelines set in place.

Other Information:

  • Appointment times have a hard start and stop time. Being late for your appointment will not extend your appointment time.

  • If any Facility CarePartners or Residents test positive for COVID-19 or community transmission reaches high risk, visits may be canceled at the discretion of the Medical Director, Director of Nursing, and Administrator. 

  • Please note that continued visits are based on your loved one continuing to meet the criteria as outlined above and as determined by our IDT Team (InterDisciplinary Team). You will be informed by the Team if your loved one does not meet the criteria and visits will not be allowed.
